Here are some forms you might be asked to complete before coming in. (Don't worry, copies are available in the office.)

The CADDRA WEISS SYMPTOM RECORD will be completed in the office

For Adults coming to see me about ADHD
The WENDER UTAH RATING SCALE (This should be completed by a parent, sibling, someone who knew you, or yourself - about you at or before the age of 12)

How to Complete the Forms

I may request a number of forms from you, and sometimes send email requests for patients to complete on-line testing and behaviour reports on the web. These usually come from iPARConnect. I will discuss these with you during our session.


Your input is very important but don't worry about answering the questions incorrectly or be concerned that you might 'label' yourself. There are no right or wrong answers. You will be asked questions on how you function in a variety of different situations. If you are unsure of an answer, provide an answer which best describes you a good deal of the time in that particular situation. Individual questions are less important than the scale as a whole, and this can only be properly evaluated by a trained professional.  


Below are resourses we often address in therapy for anxiety and depression. These are available as PDF's. To find these I suggest you type into your search engine the terms "AnxietyBC, PDF, (the title listed below)." This will lead you to a PDF that you can print and use.
